Healthy Communities

Pueblo StepUp manages a Healthy Communities program.

Healthy Communities is run by the State of Colorado and its purpose is to assist people who are eligible for Medicaid or Child Health Plan Plus (CHP+) and those who are already enrolled with questions about their benefits.

Healthy Communities is charged with a variety of responsibilities that address the “life cycle of a client” such as:

  • Generate awareness of the existence of the Medicaid and CHP+ programs through community outreach activities
  • Offer information on how to apply for Medicaid and CHP+ and provide application assistance as needed
  • Obtain a Presumptive Eligibility card so families can access Medicaid and CHP+ benefits if they are found eligible
  • Educate families on program benefits and the value of preventive health care services and how to access their benefits
  • Link clients to providers within the community
  • Provide clients with information and referrals to other community programs and resources, and
  • Explain the re-enrollment process to families that continue to be eligible for Medicaid and CHP+

In human terms, our jobs may see us talking to new moms about baby bottle tooth decay or childhood growth and development; advising pregnant teens and pregnant women about where to go for proper nutrition assistance during their pregnancy; helping families find a medical home; and working with clients who have been sent to collections due to improper billing or issues with Medicaid.

The families we help are grateful for the advice we give and connections we help them make, and that’s what makes our work rewarding.
