Getting Medicaid / Child Health Plan+

If you have uninsured children age 0-18 or are pregnant and uninsured, you or your children might be eligible for Medicaid or Child Health Plan Plus (CHP+).

CHP+ is Colorado’s program for children whose parents make too much to qualify for Medicaid but can’t afford private insurance.

Establishing Eligibility

If your income falls within published Federal Income Guidelines, you may qualify for Medicaid or CHP+. To find out if you and/or your children qualify, contact Pueblo StepUp at (719) 557-5886.

Enrolling in Medicaid or CHP+

Pueblo StepUp has qualified staff that can process your application for you. Simply contact us to make an appointment. We’ll tell you what you will need to bring with you to the appointment so that your application can be processed as quickly as possible. Call the StepUp office at 719-557-5886.

If you prefer, you can download an application for Child Health Plan Plus at the Program Eligibility and Application Kit website.

By allowing Pueblo StepUp to process your application, you may receive benefits more quickly. Pueblo StepUp has been certified by the State of Colorado to officially certify the authenticity of your documentation and then process your application directly into the State’s benefits management system. If the application is complete, we can issue you a benefits card on the spot so that you may seek the medical care you need promptly.

If your application is incomplete, we can issue you a temporary benefits card that will allow you to seek medical care for 45 days until the application can be completed and approved.
