Our Healthy Communities program will connect you to the health care and health-related resources you need, and also help you get the most out of your Medicaid or Child Health Plan Plus (CHP+) benefits.

Once you are enrolled in the program our Family Health Coordinators can help you:
- Learn about preventive health care services and how they can help you and your family
- Learn how, when and where you can access your benefits
- Link you to Medicaid and CHP+ providers that will serve as your Medical Home
- Become aware of other useful community programs and resources
- Understand the re-enrollment process so that you continue to be eligible for Medicaid and CHP+ coverage
