We help families in Pueblo City and County determine whether they qualify for either Medicaid or Child Health Plan Plus (CHP+), and if so we help them enroll and then link them to the medical services they need. We serve children of all ethnicities, ages 0-18.
Interested families can come to our offices, or use our on-site application services in both Pueblo school districts.

To be eligible for these programs families must meet income guidelines. Often children who qualify for the free & reduced lunch program will also qualify for Medicaid and CHP+ coverage.
